What is GDPR?

GDPR - Important information for Patients of Tattenham Health Centre

This Practice handles medical records in compliance with laws on Data Protection and Confidentiality (May 2018).

  • We share Medical Records with those who are involved in providing you with direct care and treatment.

  • In some circumstances we will also share medical records for Medical Research, for example to find out more about why people get ill.

  • We share information when the law requires us to do so, for example, to prevent infectious diseases from spreading or to check the care being provided to you is safe.

  • You have the right to be given a copy of your medical record.

  • You have the right to object to your medical records being shared with those who provide you with care.

  • You have the right to object to your information being used for medical research and to plan health services.

  • You have the right to have any mistakes corrected and to complain to the Information Commissioner’s Office. Please ask reception to see our Practice Privacy notices.

INFORMATION CONSENT AGE IS NOW 13 YEARS OLD

To comply to GDPR, the consenting age for information sharing is 13 years old. This means that if you are a parent/guardian of a patient aged 13 and over, we may require consent directly from the patient.

Further to this, mobile phone numbers are removed from patients who have turned 13 years old and a letter is sent directly to them requesting an updated mobile phone number. If the patient wishes to use their parent/guardians mobile number they will need to let us know this via a letter or verbally.


SAR - Subject Access Requests (Requesting access to your Medical Records)

In compliance with GDPR, you can request access to your medical record. We can give access to you via our online access request forms.

If you would like access to your medical record please complete the Online Access Request form and sign the SAR consent below. Then please bring these in to reception with your ID. Please be advised that it can take up to 28 days to grant access to your medical record.
