If you require any vaccinations relating to foreign travel please complete our Travel Vaccination Quesionnaire at least 8 weeks prior to travel and submit it to the practice. If you are travelling in less than 3 weeks, we may not be able to vaccinate you. If we are unable to vaccinate you, you will need to go to Surrey Travel Clinic where charges may apply.
Please note that if you are travelling for more than 1 month then this form will need to be submitted 3 months in advance.
The practice nurse will assess the form and any vaccinations you may require. Reception will then contact you to make the necessary appointment and inform you of any vaccine charges.

It is important to make this initial contact with us as early as possible - at least 8 weeks before you travel. These vaccines may have to be ordered if they are not a stock vaccine.
